If your insurance provider is not listed on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance company. It is possible we may be providers for other carriers besides the ones listed above. If you believe this is the case we will be happy to confirm this before examination. It is also possible that we are providers for part of your insurance and not the other (i.e. medical provider but not vision provider).

Since our office provides routine vision care and medical health care your benefits may be different accordingly. We suggest you contact your carrier to have a better understanding of your benefits before you come in. We file to insurance based upon diagnosis codes that are determined during the exam not before. This may result in an exam being categorically medical instead of routine and also vice versa. Being that many patients have different insurances for medical and routine vision this could impact the patient financial responsibility and/or their copay.
